Secure WebForm Interface

Retrieving, viewing and processing your practice forms


When a visitor to your website completes your Secure WebForm and clicks the Submit button, they're brought back to a Thank You page on your website.

That's when our Secure WebForm system goes into action. You'll receive an immediate message at the e-mail address of your choice, informing you that a Secure WebForm has been submitted and providing a link to instant access to our secure interface:

When you click on the link provided, you'll be directed to your log-in page on our secure server:

Just enter your password (we'll have already notified you of it).

You'll then see your practice's WebForm Menu:

All the Secure WebForms that have not been read yet will be shown here. Click on the Select field for the desired form.

You'll then see the filled-in form just the way it looked when your patient submitted it.

There are several choices across the top of the displayed form.

Print (longer forms may require more than one page)
Show Fields
to see just the field names and entries
Send CSV to email a file that can be opened in a word processing program
Send RTF
to email a file that can be opened in a spreadsheet program
Send PDF
to email a file that can be opened in Adobe Reader®
